Directly support case-control matching

Currently, case-control matching (ie., gender-matching, age-matching, etc.) is not directly supported. In order to extract matched cohorts, you may extract the cases and controls cohorts separately, then perform post-hoc matching.

This is a good issue, but lots of questions remain about how to best actually do this case control matching that we'll need to dig into.
