[Enhancement]: Include app title when making OpenRouter requests

What feature do you want?

When making a request to OpenRouter, it'd be cool if you could send an X-Title header to specify the title of the app making the request (i.e., "Copilot for Xcode"). I'm using OpenRouter with a number of apps, and it'd help me to figure out which app did what when looking at the OR overview table — right now it only says "Unknown" which can't be used for filtering etc.:
