Multiple choice question: reject "no box checked" as an answer

Currently, this is rejected/scored with zero with "no answer provided".

Given https://designnotes.blog.gov.uk/2021/11/15/letting-users-tick-a-none-checkbox/, allowing all-empty might be bad practice. Given that, MultipleChoiceQuestion should reject all-empty answers. (And this should be documented.)

This probably needs a deprecation period.
