Fix missing elements in nested xml and compatibility

This PR fixes #16, #21 and #22.

It is important, because without it, in some cases as indicated in #21, the iterator skips some elements.

Furthermore, it simplifies install and make it compatible with php 8 (keeping compatibility with 5.3).
