fix NoteCleanup for #12880

tested on windows, using SNoiraud's test DB, and I still get the failure when LibreOffice launches, after producing the Complete Individual Report as an odt file. "Read Error. Format error discovered in the file in sub-document content.xml at 419,116(row,col)."

also found that the 2nd and subsequent uses of the Note Cleanup tool produces these "Duplicate Accelerator" reports. (AND, Of Course, this minor issue happens with or without this PR)
