add issue tracker links to individual packages' READMEs

Consider change the READMEs in each directory under packages/ to contain links to the underlying API's service tracker, both for listing existing issues and for filing a new issue. In other words, surface the per-API support info now present (thanks to #12703 ) in each row of the main README
