Remove "Disable and Sign Out"
Description
For cloud integrations when a user clicks "Disconnect Remote Integration" the dialog contains two buttons Disable and Disable and Sign Out. They have identical result, so the second button should be removed, because it disconnects the integration in GitLens but does not disconnect it on GK.dev.
Actual result
Two buttons are presented: Disable and Disable and Sign Out
Expected result
Only Disable is shown. Remove Disable and Sign Out

We talked about it earlier and I've found it in my notes, so I'm creating an issue now. It should be fast. Does it make sense?
Yeah either we should remove it or make it actually work (e.g. sign out completely on GK.dev). I feel like the latter isn't worth it -- @justinrobots ?
