Using Agent mode but working as mixed Agent mode and Ask mode

Describe the bug

Today, after I switched from using GPT-5 to GPT-4.1, the Agent mode I was using suddenly turned into a mix of Agent mode and Ask mode. As shown in the picture, it offers the manual insertion feature from Ask mode, but at the same time, it still uses Agent mode's tool for correcting errors. However, the automatic code modification feature has disappeared.
