Recommend an FAQ / Troubleshooting section

I think we should make another .md file like FAQ.md, to be linked from the main README.md. We could put some of the more common answers there and close them out from the GitHub Issues tab. Things like steps to try to recover from init failures, how to have the virtual controller replace the physical controller already attached to a gamertag (in response to the "nothing happens" questions), etc. Also I think we should hide the GitHub Wiki tab through the GitHub settings since it's blank and I think it makes more sense to just have such details paralleling the code.

Review linked issues for info we should collapse to a FAQ.
