[Feature] Issue forms instead of issue templates

GitHub has a feature to make issue forms instead of more basic issue templates.

I think using it here may be beneficial. Instead of having 1 text field pre-filled with information that people can immediately submit without editing (which most people do), forms allow the content of the issue to be divided into multiple sections, some of which can be prefilled based on url, while others left blank and marked as required, blocking submission until something is entered there.
