Feature request: For the open TSP a start and endpoint can be specified

It would be beneficial to be able to specify the start and endpoint while solving the open TSP for a given distance matrix. As minimal solution you could also extend the readme for a further 'trick' solution: By extending the distance matrix with a dummy point connecting the start and end points with zero weight, but all others with an inf weight.
