Fixit + Inline coder + scroll = duplicate

To reproduce:

1. Create a method with an unreferenced self message and save it.
2. Click on the fixit-icon and create the method (results in an inline code)
3. Scroll so that the coder goes off the screen
4. Scroll back

Result: duplicate coder that cannot be removed.
