Improve presentation of document and annotation pages

Please update placement of Annotation and Document View pages:

1. move Assigned document labels to the top of the page
2. transform "tabbed" layout of Categories tab to simple list to avoid additional clicks

Having some confusion implementing the second point. If tabs are removes and all three categories: Layout, Links and Segmentations are displayed at the same time as a simple list, how should Annotation type should look like? Because all of them have different one.
