New Items always going to "Always Hidden"

Sequoia 15.7.1 Hiddenbar 1.9 (13)

Hi, I'm finding that when new menubar items are added that they are always being put in the "Always Hidden" section rather than in the main section. Is this a known issue (or feature) or am I doing something incorrectly?
