apiref uses old DITA-OT XSLT

Converted from SourceForge issue 2018375, submitted by debbiep-sf. Migrated from dita-ot/dita-ot#1084

apiref and javaapiref have calls to the "flagit" template, which is deprecated in recent DITA-OT. These uses should be rewritten to use the replacement DITA-OT code.
