Index Management in the Schema Designer

Currently the indexes in the schema designer are not actually well implemented.

[x] The index should be displayed by name instead of the CreateIndex { .. } notation (via https://github.com/digitallyinduced/ihp/pull/762)
[ ] One should be able to right click the index to delete or rename the index
[ ] One should be able to create a new index (right click → Add Index)
