Add quake iv support.

Implement the ability to run quake 4 like for example here

Started looking at what work was needed for this earlier this year - with it being the Quake 4 SDK as a baseline and Doom 3 code applied on top its not as straight forward as hoped but will try and find time over summer to look further but good to find out if others are looking. Will be great to get working.
