Please check your error log php... I think your problem is php image library, and what kind of image library do you have?

I find problem solve. when is use Layout Libraries then problem.

I got same problem is bercode generate in CI Framwork. when i delete Layout Libraries then issue is solved.

@mysoftit I am using AdminLTE, besides that I am using a library for QR Code. The QR Code is working fine
