Add Title for Proposals For the user entry modals to create proposals

With pre-populated default title that is instructive enough of the type of proposal it is, that the user can change if they want to.

@magentaceiba These are the modals that need to be changed:

In addition to the title input boxes, the batch transfer option doesn't have the grey background anymore. Also, (not sure if Fabiola has updated these yet) the proposal fee is now to the left of the submit button. The texboxes, drop-downs, etc now have a border radius of 8px instead of 4px.
