Looks like plugin/cr was deleted, where's a good place for the remaining APHL code? Or should we resurrect plugin/cr?

Most of it should be upstreamed to clinical-reasoning. After it's there (I see quite a few KnowledgeArtifactVisitors) we'll expose those operations in hapi-fhir-jpaserver-starter directly. There's a little bit of manual process to do that still.
