Standardize tooltip for chat section
Description
Before this fix, tooltips were showing for some items and not for the others. The inconsistency fells like a bug. In addidtion, the format of the tooltips doesn't match the standard pattern in VS Code.
This fix provide tooltips for all the items in the Continue UI and formalize the text format of each tooltip. https://github.com/continuedev/continue/issues/4819

At the time we decided to close that one but realized it would nice to have status information in the tooltips so I redid it here https://github.com/continuedev/continue/pull/5343 but same idea

@Jazzcort looks like I don't have permissions for this branch, if you can fix merge conflicts (basically take out the notch section tooltips I think) then this is good to go!
