REDO: Modify UNIQUE constraint on xids table

To not force a block based on @joshsmith2's capacity, redid PR as per @metasoarous comment in https://github.com/compdemocracy/polis/pull/758#issuecomment-822894861
cherry-picked @joshsmith2's initial commit so that he gets credit in commit history and enjoys great glory in the afterlife 🙂 (assuming one subscribes to a belief system in which this is what's tallied)
Just a note here that on our local installation of Polis we have had to apply these changes to get xid's working as per the original conditions which raised this PR.
I'll update with some feedback soon. So far we have noticed some weird behaviour with postgres container having a crash, and some existing conversations breaking completely. We simply created new ones and are proceeding [as xids are a hard requirement given the way this is being embeded in our research project].
I haven't had a chance yet to dig too deep yet, and it could be due to some configurations specific to our install. But will let you know how it turns out once we get some more users involved and I have some time to look further.
Note: Had to add an additional commit to make this fully idempotent.
