If the model already exists but isn't being recognized, try disabling all custom nodes and testing again.

How do we know whether to put into /checkpoints or into /diffusion_models? What's the difference? Do we really need both?

I think this is a very pointless and "academic" distinction. For good UX these folders can and should be combined as there are no drawbacks, only less friction for end users. I have just symlinked them.
Some checkpoints might contain CLIP, VAE, and diffusion_model all in the same model—more like an all-in-one. You need the load checkpoint node to load and use them.
Many diffusion models are not contain CLIP or VAE, so you need additional VAE and Clip models.
If we mix them in the same folder, users may sometimes encounter errors and find that some models do not contain VAE / CLIP models.
Also, some models are using the same VAE and Clip. Sometimes the model files are very large, so you might not want them combined in the same checkpoint model, as this will take up a lot of your disk space.
