Visits UX improvements

Basic Information

• "Visit" nav button should be renamed "Visits" to keep pluralisation consistent with "Contacts"
• Keep margins around action buttons within expanded visit view - they're currently lost in mobile views per attached.
• After "Submit"ting an incident or pet concern, close the incident/pet concern view.
• Add visual indicator to visits that have an Incident or Pet (/Vet) Concern recorded.
• Notes view should "select distinct". If there was a change like "Incident reported", mention it in history.

I am looking at the code here and I have a question for you:

About the visual indicator improvement, could be an icon in the Visits area?

• After "Submitting an incident or pet concern, close the incident/pet concern view. - This issue is working in the main development, maybe need to upload to the site.
