Feature Request: Easy way to add customized fields

I don't know if is possible in the actual state of the gem, or just add proper documentation to it.

For example a combined field to show:

Something: ________________ Date: __/__/____

• [ ] Add Documentation
• [ ] Add Tests
• [ ] Add Proper Code
