Make more resilient : the caching of UMLS / NCI codes

If the CVS system holding the metadata UMLS and NCI codes is unavailable at cache time, we (and the CVS maintenance team) should get warning messages. If we don't get a refresh of the cache, perhaps we should maintain the previous cache values until we do .. and we should start polling more frequently until the CVS server comes back on line.

We have had at least two "outages" where the CVS system was unavailable, so we should be prepared to handle this possibility going forward.
