Merge PSGI

Hey @book, could you merge the PSGI branch by chance?

[...Years later...] It was not possible to change Act back in 2014 (even if the reasons are not clear to me). I'm looking carefully at this branch with the objective to make it live.

We're actually using PSGI branch (with some modifications) for Russian conferences - see e.g. http://event.yapcrussia.org/ryazan2018/ and others at the same platform. I'm going to publish my modifications soon - hopefully it'll be easier to make them alive then.

Just as an FYI: I've a PSGI based rebase in #73 there were some merge conflicts that I resolved and it also includes #69.
