The idea is to not mix `begcaps`/`endcaps` with `begcapsword`/`endcapsword` and `begcapsphrase`/`endcapsphrases`. This is explained in the manual for the emphasis opcodes.
Yes, but that is happening because the rules are mixed. The implementation assumes that the rules are not mixed, if that assumption is not met, it outputs rubbish.
