No execution file any longer

I don't know if i am the only one who encounters this

But there is no ConfigurationForm.exe file and the older versions dont work

As in within the release zip here? I haven't changed anything with these in years, so I don't think there's anything wrong. Best I could guess is that the exe is flagged by your anti-virus and is auto-deleting it when you extract the zip file.

Thank you for the quick reply, apparently i was downloading the wrong script xd

I managed to open the exe with your link (thank you)

But the program does not work. It doesn't calibrate when i move my stick, even after following through with the steps in the video

Perhaps the program doesn't support PS4 controller and i have somehow missed this?

Hmm, yeah if there's a way for you to make the controller XInput then it should work. Otherwise no. I'd try using DS4 Windows first and if that doesn't work, try using Steam to remap the buttons maybe. The script is just reading XInput, so any controller that sends that will be detected.
