Urgent: Name Examination UI - comment field not populating

When Name Examiners Edit the Name Request a comment was inserted into the Comments table. This functionality worked on December 3rd in PROD.

Today Claudia changed the name choice for this NR and it does not add a comment anymore: NR 1904410

I reproduced this error in test, something has broken this process probably in dev/test main branch. NR 1307439

changed name to ditto INC. No comment found.
