@BrandonSharratt This form is too wide as you've got it. I think we need to size it like we have the righthand rail - it's ok there's blank space there. So the max width would be approx 787px wide or whatever you have set for that main content block when there's a righthand rail. image.png

And then for the submit button I think we need to follow the same pattern as the rest of our filings with the submit being on that white bar at the bottom: image.png

It should say: 'Submit request'

@BrandonSharratt , Its showing "Submit "instead of "Submit Request" as per the design. Can you pls check?
