Legal API: continuation in review email templates

Design ticket

Create/update templates for the following emails:

1. user has initially submitted the continuation in application
2. staff has reviewed the application and requests a change (*)
3. staff has reviewed the application and accepted it (*)
4. staff has reviewed the application and rejected it (*)
5. the filer has completed the application
6. If Reject - Trigger an email to IT OPS with standard format for Refund- Need to check

The emails marked with (*) need to include the email body text from the staff review.
