@chdivyareddy Just a heads up, this form isn't available until implemented in the subsequent tickets, so i'm not sure how this specific bit will be QA'd :)

You can see the form fields in the PR pics, if that helps otherwise let me know what youd prefer here, thanks!

Verified the common location form and the max character validations by looking into the PR pics. Will verify in the UI once they are ready!

@chdivyareddy I'm wondering if it makes sense to move this to done so it doesn't sit. The UI tickets may not be tackled right away (they are linked to the third sprint goal). Since those UI tickets will need QA I'm sure if anything was awry we'd identify if then and could address.
