Update Nyquist plug-in names and headers for consistency.

Removed obsolete ;action header. Removed ;manpage header for plug-ins without GUI. Bump ;release patch number. Fix other minor consistency issues.

Resolves: Inconsistent capitalization of effect names. Resolves: Obsolete headers.
