RAT-465: Removed all deprecated code and options

DO NOT MERGE

This is the PR to move the FEATURE branch that removes deprecations into master.

This PR will have a massive number of changes and be rather large, thus is is setup as a FEATURE branch so that we may review the changes in smaller chunks.

@ottlinger Please review but do not approve or merge. This is the base change against which fixes for broken tests and functionality will be made.

@Claudenw thanks for all your work - seems like a good direction to me and could still be a 0.18 or 1.0.0 in the long run. Thx for all your efforts so far.
