"Plan Mode Execution Failure: No Action Taken After User Approval"

Bug Description I've noticed an issue when using plan mode where Claude doesn't actually execute the proposed plan after I approve it. Specifically, after I choose "yes" to agree to the plan, Claude sometimes just returns a simple text response. In its message, it claims to have completed the tasks we agreed on, describing what it supposedly did, but it doesn't actually perform any of the actions—no code is written or changed. The session then ends as if the work was done, which can be misleading.
