[Bug] Claude randomly exits to shell during file edit/create operations

Bug Description Claude exits to terminal at random times when trying to edit a file or create a file.

Image Before the `/feedback`, it exited to terminal and I had to launch Claude and and resume this conversation. It was about to present me the plan.
