Improve/create proper documentation (README is too big)

I really think this Ansible role could so with some more documentation. I will try and contribute some when I get it working for my setup.

While we're at it - example repos for deploying and rolling back popular frameworks / languages could really help this project get traction. I'd be happy do contribute examples for php/wordpress, node.js/express and python/django. It's the least I could do considering how great ansistrano has been to me.
