Citation Metrics report - add clarifying info

After having generated the Citation Metrics report, it would be nice to have the option to add some clarifying text. When you currently print the report, there is a header NASA/ADS Metrics Report with a URL underneath. One option could be a text box you can click to activate and type in some information (like Metrics Report for Mike Kurtz) or a button Add info, bringing up dialog to specify info, which on submission gets added under the report header.
