Two verification code boxes appear on the page when one is expected

I just asked for two IndieAuth codes in the space of about a minute. I went back a page and then back on to the IndieAuth page. When I pressed the button to ask for a code, two input fields appeared. I have seen this behaviour multiple times as I have been testing my IndieAuth.com integration.

Could you please investigate this issue? Having two input fields appear to users is confusing as it is unclear: (i) why there are two fields and; (ii) in what field one must enter their access code to verify themselves.
