Settings > Login Items: app is missing icon and shows developer name instead of app name

Open Settings and navigate to Login Items, observe that Xcodes shows up without an app icon and the name of the app is "Matt Kiazyk" instead of Xcodes. This sort of sketched me out so I had disabled it until I remembered who and what a nice guy Matt is ;)
