Form Data Menu Item

Form Data should be in the top-level navigation - not part of agencies. As per the design at https://mongodb.invisionapp.com/share/VJXLVXNPGW4#/screens/425495472_Agency_Admin-Form_Data-Activity-None

The global administrator view should stay the same (part of the agency) as designed at https://mongodb.invisionapp.com/share/MYXRXC2T2H3#/screens/425454264
