Outdated Docs (& Issues with WP 4.9.5)

Hey @kadamwhite, me again! Was struggling to get the plugin working (on WP 4.9.5; was installed, activated, but failing without any errors...). Then I happened upon @danielgcnett's comment on issue #26. He linked to a fork which does seem to solve my issue.

That said, maybe you can add a link in the README? If only to simplify the recon process for others! Especially since this main repo seems to get a lot of in-links from around the WP Rest API world... In any case, again, amazing work on all these plugins!
