Infractions System (s!warn)

What

We'd have a s!warn <user> [reason] command, which would record an "infraction". Infractions could then be looked up using s!infractions <user>.

Why?

This system would be the foundation for other automatic moderation systems, like anti-links or anti-invites, which would warn users once they break the rules.

Then, we could create a module that lets server administrators to configure automatic actions to be taken once a user reaches a specific amount of infractions in some set period of time (e.g. if the user has two warnings in less than 10 minutes, ban them)
