Ensure Pytorch/TF consistency for HiddenOutput class

At the moment the pytorch version applies up to the indexed layer whereas the tensorflow version applied the indexed layer as well.

@ojcobb I believe the subclassed PyTorch models are also restricted on arbitrary ordering of the layers at init which might restricted the HiddenOutput functionality.
