Error message when creating new contact, name prefilled as existing case person

Bug Description

It seems that when you create a contact from the case form, the name is prefilled as the case person instead of allowing you to enter the new contact person's name. additionally, when you fill out all the other information, an error message appears. Probably because its a mistake to have the new contact have the name pre-filled as the case person's name

Reproduction Steps

1. Login
2. Click on existing congenital rubella case
3. Click on create new contact (from case person form)
4. Enter all information on screenshot- phone number, email, not a returning traveler, report date (tried 2022, tried 2025), disease of source case, date of last contact, responsible region, responsible district, type of contact, relationship with case
5. Error message appears

Expected Behavior

I should be able to create a contact person and enter their name from the case form. I should be able to save the contact without an error message
