Background Many users feel that selecting from a list of choices to answer the COI questions will make for an easier and less stressful questionnaire experience. An "other" section should be available at the bottom of each question for people that wish to elaborate.

Describe the solution you'd like Connie and Kim of VNP will draft a template of the select options questionnaire by next week and share with our team. Tasks: Implementing the multiple choice version of the questionnaire instead of the text box, making sure that users can select multiple options, and making sure the concatenation of all selected options and answer to the "other" section are submitted together into the database.
