Add Markdeep style guide

To ensure the books are rendered consistently on web (and in print), we need to adhere to some Markdeep style.

Creating a Markdeep style guide will ensure all of the "hidden" rules are documented in a single place, and can be visible from the CONTRIBUTING.md guidelines.
