Remove blade pitch actuator from BeamDyn and blade PitchAxis from ElastoDyn

Feature or improvement description This PR removes the incomplete blade pitch actuator implementation in BeamDyn. This functionality has been superseded by the blade-pitch DOF in ElastoDyn and the pitch actuator model in ServoDyn (PR #3039).

The unused blade PitchAxis input column is also removed from ElastoDyn. The legacy 6-column distributed blade property input table is no longer supported. From now on, the blade property table must have 5 columns with PitchAxis removed.

Test results, if applicable No change to existing test results after removing the Pitch Actuator section of BeamDyn input files and deleting the PitchAxis column from ElastoDyn blade input files.
