Include Issue assignment process in README

Currently, in the README, it shows how to fork, create a branch and do the changes and then create Pull Request. It doesn't mention that the issue should be assigned first before opening a PR which is given in here. So we need a paragraph in the README strictly mentioning that it should be assigned, following all the contributing guidelines. Also, modify the 4th point in the contributing and mention the good commit briefly that is given in here.
